Description

This variant has been observed in individuals with cystic fibrosis (PMID: 9401006, 23974870, 26911355). In at least one individual the data is consistent with the variant being in trans (on the opposite chromosome) from a pathogenic variant. This variant is also known as 1259insA and c.1127_1128insA in the literature. ClinVar contains an entry for this variant (Variation ID: 53197). For these reasons, this variant has been classified as Pathogenic. Loss-of-function variants in CFTR are known to be pathogenic (PMID: 1695717, 7691345, 9725922). This variant is not present in population databases (ExAC no frequency). This sequence change creates a premature translational stop signal (p.Gln378Alafs*4) in the CFTR gene. It is expected to result in an absent or disrupted protein product.

Description

Variant summary: CFTR c.1130dupA (p.Gln378AlafsX4) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant was absent in 250100 control chromosomes (gnomAD). c.1130dupA (aka. 1259insA or c.1127_1128insA) has been reported in the literature in multiple individuals affected with Cystic Fibrosis (e.g. Shrimpton_1997, McGinniss_2005, Sosnay_2013). These data indicate that the variant is very likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Three submitters have provided clinical-significance assessments for this variant in ClinVar after 2014, and classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic.

Description

The c.1130dupA (also known as c.1127_1128insA and legacy c.1259insA) pathogenic mutation, located in coding exon 9 of the CFTR gene, results from a duplication of A at nucleotide position 1130, causing a translational frameshift with a predicted alternate stop codon (p.Q378Afs*4). This mutation was originally reported in an individual with pancreatic insufficient cystic fibrosis with a second CFTR mutation (c.1585-1G>A, legacy c.1717-1G>A) detected in trans (Shrimpton AE et al. Hum Mutat., 1997;10:436-42). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.
