Hypothetical model for tumor antigen-based vaccine efficacy. Tumor antigen vaccines given subcutaneously, intralymphatically, or intranodally are designed to deliver tumor antigen-derived epitopes into secondary lymphoid tissues (ie, vaccine-draining lymph nodes, VDLNs) in an immunogenic format (Step 1). At the current time, this may best occur via the use of patient dendritic cells (DCs) in the vaccine, or by applying the antigens in adjuvants that promote antigen-processing by DCs in situ. Primed or boosted tumor antigen-specific T cells or specific antibodies then recirculate throughout the body and may infiltrate the patient's tumor lesion(s) (Step 2). If these vaccine-induced T cells or antibodies can effect tumor destruction in situ (Step 3), tumor debris containing tumor antigens may be acquired by DCs and prime/boost specific T cell/antibody responses in the tumor-draining lymph nodes (TuDLNs, Step 4). Like the initial vaccine-induced T cells and antibodies, these TuDLN-derived T cells and antibodies may also infiltrate the tumor lesion and promote regression. If steps 2 and 4 are repeated and of sufficiently great magnitude and antitumor function (Step 5), objective clinical responses may be observed.

Different diagnostics such as ultrasonography, for example, to measure size (C), optical coherence tomography, for example, to detect subretinal fluid (D), autofluorescence to distinguish drusen from orange pigment (E), and angiography to detect tumor vasculature (F) are useful to classify the lesion.
