This figure depicts a sample growth chart, developed by the Centers for Disease Control and Prevention, for clinicians to use to plot the growth of female children and adolescents ages 2 to 20 years old. At the top of the form is a place to record the patient’s name and record number. Below that is the growth chart, which plots the patient’s body mass index (BMI) by age. The x-axis is age in years, starting at 2 years and progressing to 20 years in increments of 1 year. The y-axis is BMI in kg/m2, starting at 12 kg/m2 and progressing to 35 kg/m2 in increments of 1 kg/m2. Between the ages of 18 and 19 years the chart notes the location of the 5th, 10th, 25th, 50th, 75th, 85th, 90th, and 95th BMI percentiles to allow comparison with other girls of the same age. In the top left of the chart there is a blank table where clinicians can record the date; the patient’s age, weight, height, and BMI; and any comments. Below the growth chart is a table depicting the BMI standard deviation score (SDS) for selected percentiles. For girls aged 8 years old who measure 50.5 in (128.3 cm) tall (50th percentile for height), the data for the 50th percentile is the following: weight of 57.2 lb (26.0 kg), BMI of 15.8 kg/m2, and a BMI SDS of 0.0. For the 85th percentile: weight of 66.3 lb (30.1 kg), BMI of 18.3 kg/m2,  and a BMI SDS of 1.0. For the 95th percentile: weight of 75.0 lb (34.0 kg), BMI of 20.7 kg/m2, and a BMI SDS of 1.7. For girls aged 12 years old who measure 59.5 in (151.1 cm) tall (50th percentile for height), the data for the 50th percentile is the following: weight of 90.9 lb (41.3 kg), BMI of 18.1 kg/m2, and a BMI SDS of 0.0. For the 85th percentile: weight of 109.0 lb (49.5 kg), BMI of 21.7 kg/m2,  and a BMI SDS of 1.0. For the 95th percentile: weight of 126.6 lb (57.5 kg), BMI of 25.2 kg/m2, and a BMI SDS of 1.6. For girls aged 16 years old who measure 64.0 in (162.6 cm) tall (50th percentile for height), the data for the 50th percentile is the following: weight of 118.7 lb (53.9 kg), BMI of 20.45 kg/m2, and a BMI SDS of 0.0. For the 85th percentile: weight of 143.1 lb (65.0 kg), BMI of 24.6 kg/m2,  and a BMI SDS of 1.0. For the 95th percentile: weight of 168.1 lb (76.4 kg), BMI of 28.9 kg/m2, and a BMI SDS of 1.6.
