Abnormal sequence of activation in explaining QRS patterns. The normal sequence of activation is diagrammed in Figure 33.8.

  1. Right bundle branch block. Vectors 1, 2, and 3 remain normal. Vector 4 explains the R′. right ventricular depolarization tacked on to the otherwise normal QRS.
  2. Left bundle branch block.
  3. Left anterior superior hemifascicular block (left anterior hemiblock).
  4. Right ventricular hypertrophy.
  5. Left ventricular hypertrophy.
  6. Anterior myocardial infarction. The resultant vectors are all directed away from the infarcted area so that the exploring electrode looks through the electrically silent "window" into the lumen, to record negative complexes.
